The Cardiovascular application captures comprehensive data and clinical results of various cardiac procedures. These include CABG, valve replacement and CRM device placement during a 5-year follow-up period. It also incorporates in-depth cost-driver information providing better assessment of the economic impact of new procedures and products.
The PhDx® Cardiovascular application consists of three modules:
- CRM Device
- CRM Patient Regsitry
- CHF General Outcomes
Whether independent of each other or in harmony, reports for each module are available to measure patient and program outcomes in four areas: clinical, patient health status, patient satisfaction, and cost drivers. All information is stored in a single database repository, convenient for all stakeholders.

The following survey instruments are included in various modules:
- Cardiovascular Patient Information: The survey collects patient demographic information and baseline cardiovascular functional measures.
- Echocardiogram: This is an ultrasound test procedure (often called "echo") that is a visual recording of the heart's movement. The test is used to evaluate a number of cardiac conditions.
- Minnesota Living with Heart Failure
questionnaire: This survey collects information on how patients are affected by their heart condition.
- SF12v2SM: The survey measures eight categories of physical and emotional health, which indicates the impact poor health and pain have on physical and social activities.
- Six Minute Walk Test: The survey measures the distance a patient is able to walk within a six-minute period as well as records any symptoms they experience.
- Initial Clinical Assessment: The survey includes questions about the patient’s clinical condition, including medical history, clinical measures such as EF and current medications.
- Procedure Summary: Collects data regarding the procedure performed and cost driver information. Also includes device information (if any) and radiographic report on implant.
- Follow-Up Clinical Assessment: Collects and reports information on the patient’s clinical condition such as medications, any post-operative complications, and device function (if applicable) at the time of visit.
- Patient Satisfaction with Services: This survey asks about patient satisfaction with the care received at the facility where the surgery was performed.
